At MMH, our transitional care services are designed for patients recovering from an illness or surgery who no longer need hospital-level care but are not ready to go home. Our program is called transitional care because its focus is to help you or your loved one transition to the highest level of independence. Transitional care services can also be known as swing bed programs.
Because transitional care is provided within our inpatient unit located on the second floor of our hospital, our patients benefit from having convenient access to our lab and radiology services, as well as our comprehensive care team. Together, our caregivers create a personalized plan unique to each patient’s needs. Additionally, the team holds regular bedside rounds with patients and their families to ensure everyone understands the plan of care. Length of stay is determined by patient progress.
Our transitional care program is supported by evidence-based best practices through a partnership with Allevant Solutions developed by Mayo Clinic and Select Medical. The program is predominantly covered by Medicare and Anthem Blue Medicare.
